Cut down on wait time at the Office by printing out these forms beforehand and completing them at your own leisure. You may even be able to save yourself a trip to the gym! ALL OGA SPECIAL EVENTS (INCLUDING CAMP, PARENT'S NIGHT OUT, AND SLEEPOVERS) REQUIRE PRE-REGISTRATION AND PAYMENT (CASH or CHECK ONLY). SPACE IS LIMITED. Each of the forms below will open in its own browser window.

• Registration & Release of Liability Form (2 pages) - The form is in fillable PDF format. Be sure to complete both pages before printing.


• Adult Participation Form - For parents (or legal guardians) who will be on the gym floor for our Tots classes.


• Special Event Registration & Release - For non-members (kids NOT enrolled in classes attending Field Trips, Sleepovers, Camps, Birthday Parties, etc.)


• Employment Application - OGA is always keeping an eye out for new additions to our wonderful staff!
